Working in Belgium - Type B work permit or professional card
Documents to be submitted with a visa application:
When applying for a visa, you need to present the following documents at the
embassy or consulate responsible for your place of residence:

A valid travel document (e.g. international passport) valid for one year after the visa issuance date.
It is advisable that your passport contains at least two double blank pages reserved for visas (blank front and back) for each applicant (so, if a child travels with parent in the parent’s passport, there should be 4 blank pages (blank front and back), i.e. 8 blank pages.

Two visa application forms
correctly completed, signed and accompanied by two recent passport
photographs
bearing a true likeness to the applicant.

Your original valid type B work permit (permis de travail B/arbeidskaart B)
(that your employer in Belgium must request) / original professional card

An extract from your judicial record (Police Clearance) not more than six
months prior to the date of application and covering the last 5 years.

A medical certificate filled out by a Belgian embassy or consulate approved
medical centre not more than six months prior to the date of application (click
here to see the example). The medical certificate is not necessary if you have submitted a valid type B
work permit.
- Copy of the internal passport: personal information (page 3), military
duty (page 13), Marital status (page 14,15), Children (page 16, 17), others.

Documents should be submitted in original + 2 photocopies. All originals are
returned.
Important :

The documents listed above are only the basic documents to be submitted in all
cases. Additional documents may be requested by the embassy or consulate.

Official documents issued in Russia or in Belarus, Georgia, Armenia, Kazakhstan, Uzbekistan, Tajikistan and Kyrgyzstan must be apostilled/legalised and accompanied by a certified translation.

The above mentioned type D visa is a one entry national visa and requires
registration in the Belgian population registers on arrival.
